New Patients

Please call into the surgery to Register.

When registering please note that the Surgery DOES NOT have any female GPs.

To register with us you must visit reception. All new patients must complete registration forms 1 and 2. If registering a child under 5, please also complete form 3. All are available to download below. Our receptionists can help you to complete the registration forms.

We mainly accept patients living in the Sheffield 11 area but our practice area also covers some parts of Sheffield 7, 10 and 17.

All patients will have a named GP responsible for their care, but they can choose to see whichever doctor they wish.
